ITS believes this to be related to patch KB3045999. It has been found that where this issue can be replicated, removing this patch fixes the issue. While a root cause cannot be determined at this time, ITS suggests comparing patches installed on affected systems and non affected systems. ITS also suggests reviewing and testing all patches prior to deployment in production environments.
